Facility Use Form

This page is for anyone looking to rent the building in any capacity. Those looking to rent the facility outside of community hours must first email Keith Myers or Gage Morgan at gmorgan@lhsd.k12.oh.us to check availability. The form attached below will then be filled out by the renting party, as well as Keith Myers to create the contract between the renter and the Logan Hocking School District.

5:00 pm – 7:00 pm
Fri-Sun Closed for Events

  • Check the Website for Holiday Hours When School is Not in Session
  • 2 Hr School Delays/Early Dismissals/Closings: the Chieftain Center Will Also be Closed

Adults must sign the waiver form and the adult application below.
Students must complete the waiver form and the student application below.
The forms can be scanned and emailed to gmorgan@lhsd.k12.oh.us or returned to the Chieftain Center Front Desk

What's available for you!

People living within the Logan-Hocking school district will have access free of charge pending application approvals.

Anyone residing outside of the Logan-Hocking school district who wants to use the facility will have access through a registration and payment agreement.

Registration for Chieftain Center access will include the use of the following:

  • Basketball Courts 
  • Volleyball Courts 
  • Tennis Courts 
  • 200m Track for Walking
  • Turf Field
  • Golf Simulator (Sessions will be assigned via email.
  • Only Sign up for one session per week. Email: kmyers@lhsd.k12.oh.us)

All courts will be put on a rotating schedule based on the utilization of the courts. The schedule is subject to change, pending usage demands.
